HIPAA Notice of Privacy Practices

Our commitment

This Notice describes how medical information about you may be used and disclosed, and how you can get access to this information. Please review it carefully. Organic Well is required by law to maintain the privacy of your protected health information ("PHI"), to provide this Notice, and to abide by its terms.

How we may use and disclose PHI

Treatment: to provide, coordinate, and manage your care, including with laboratories, pharmacies, and other providers involved in your treatment. Payment: to bill and collect payment for services, including providing superbills at your request. Health care operations: for quality improvement, training, and practice administration. Other uses permitted or required by law include public-health reporting, health oversight, and legal compliance.

Uses requiring your authorization

Most uses of PHI for marketing, any sale of PHI, and most sharing of psychotherapy notes require your written authorization, which you may revoke at any time in writing (except to the extent we have already relied on it).

Wearable and third-party data

If you choose to connect wearable-device data (for example via Heads Up Health), that data becomes part of the information we use to guide your care and is protected accordingly. You may disconnect data sharing at any time.

Your rights

You have the right to: inspect and obtain a copy of your PHI (usually within 30 days); request corrections; request restrictions on certain uses and disclosures; request confidential communications by alternative means; receive an accounting of certain disclosures; obtain a paper copy of this Notice; and be notified of a breach of unsecured PHI.

Our duties

We are required to maintain the privacy and security of your PHI, notify affected individuals following a breach of unsecured PHI, and follow the terms of the Notice currently in effect. We reserve the right to amend this Notice; the current version will be posted at our clinic and on this website.

Complaints

If you believe your privacy rights have been violated, you may file a complaint with our Privacy Officer at (305) 930-8891 or with the U.S. Department of Health and Human Services, Office for Civil Rights. You will not be penalized or retaliated against for filing a complaint.
